If you want to try to run it yourself, you'll have to edit the .htm file to change the name of the server the response is sent to AND the name of the managed system (within the SOAP request) for which the query is being made.

ITM 6.1 SOAP Example
Here's an example SOAP request from within an HTML file. The file uses JavaScript to make a SOAP request to the CMS server and reads the hello1.xsl file to process that request. It's something that can hopefully get you started.
